A Catalyst Transition plan is a visual tool used to prepare young adults for leaving school and taking on new responsibilities and roles.
As a young adult, you may have many different plans that have been created for your future. Catalyst Transition planning brings these together in an overarching plan for the life you want.
This planning tool will help you think about what you want and keep you on track and motivated to achieve your goals. It will involve your family, friends, teachers and other people you choose.
How does Catalyst Transition planning work?
ImagineBetter will first set up a planning meeting with you. You can pick the date, time and location. Often people feel more comfortable planning at home.
You can invite your family, friends, teachers and other people to your meeting. Make sure you ask them well in advance.
A Catalyst Transition planning meeting is a guided conversation and takes between two and four hours. Two ImagineBetter facilitators will guide you through the process.
The Catalyst Transition process
A Catalyst Transition plan might include topics such as:
- further education
- moving out of home
- friends and relationships
- your health
- your culture
- things you do for fun
- contributing to your community.
At the end of your planning session, you will be presented with your visual plan. You can also ask for your plan to be turned into a written report. This will include the discussion behind your plan.
Catalyst Transition planning costs $125 per hour plus GST, up to a maximum of $500 plus GST.
Travel is an additional cost. This can be negotiated when setting up your planning meeting.
If you would like to use Individualised Funding (IF) to cover planning costs, talk to your IF coach before confirming your planning meeting.
Please be mindful that you cannot use IF to cover costs, unless you are already signed up to this type of funding at the time your plan is created.
